Payment Terms- Amount once paid is non-refundable.- You agree to pay the amount declared by the clinic including additional charges imposed on Sundays or holidays.- No discount, unless otherwise mentioned, will be provided.- In case a patient produces any previous reports after the scan, we’ll have to redo the scan at full cost. Report Terms- All previous reports and relevant documents should be provided to the doctor (even if taken at other centers) in order to get the full picture. Failure to do so will end up in incomplete information and neither the doctor nor the staff will be liable in case of issues in the future.- In case a patient produces any previous reports after the scan, they’ll have to redo the scan at full cost.- Patient details entered here will reflect on the report and will not be changed.- Blood test reports are dependent on the third-party lab. We will not be held responsible for any delays or mistakes.- It is possible that certain things may not be detectable in ultrasound scans for various reasons. Neither us nor our staff will be liable.- Neither us nor our staff shall be subject to any legal action or lawsuit. Behavioural Terms- Basic etiquette is expected of all patients and their attenders. Misbehavior of any sort will not be tolerated.- Patients need to clean up after themselves.- Please ensure your children are well behaved. This is a healthcare center and children could be exposed to infections if they aren't supervised closely.- We retain all the rights to deny service especially based on terms of behaviour. Scanning Terms- Gender will not be revealed.- Attenders will not be allowed inside during the scan/ procedure.- No capturing photos or videos of the scan during the procedure.- During pregnancy scans, the fetus should be in a certain position for the scan to be completed. We do not do incomplete scans.- Patient’s consent is absolute. No scan or procedure will be performed against the willingness of the patient. Follicular Scan Terms- 1st scan of every cycle will be Rs 1500 (before day 5 of menstrual cycle) or Rs 1000 (after day 5 of menstrual cycle).- Follow up scans for this menstrual cycle will be Rs 500 per scan.- Measurement and size of only 1 dominant follicle on each ovary, as applicable, will be measured and provided in the report.- There will not be any refund or adjustment with respect to number of scans performed.
